Form DS-11, Application for a U.S. Passport

Citizens of the United States, and non-citizen U.S. nationals, can apply for a U.S. passport or a U.S. passport card. Both the regular passport and the passport card shows the bearer’s identity, and nationality, as well as where he or she is originally from. The Passport card can be used to cross the borders between the United States and Canada, Mexico, The Caribbean and Bermuda. The passport card cannot be used for air travel. The regular passport is required for all international air travel, and is accepted at border crossings all over the world. To apply for a U.S. passport, applicants must show proof of U.S. citizenship and be able to prove his/her identity to the passport agent.
