Form I-90, Application to Replace Permanent Resident Card

Permanent residents in the United States who need to replace their permanent resident cards (green cards) should fill out Form I-90, “Application to Replace Permanent Resident Card,” and submit it to the United States Immigration and Citizenship Services. Form I-90 should also be used to permanent residents who need to renew their old green card. The I-90 form can also be used to update personal information, for instance a change of name. Conditional permanent residents may also use Form I-90 to renew or to replace their resident cards, but Form I-90 cannot be used to apply for the removal of those conditions, or to extend the residency status.
